For thirty years, security has been optimized to protect an organization. A breach is measured in dollars, downtime, and reputational damage — painful, but institutional, and increasingly insurable. Companies write it down, notify the affected, and recover. The entire apparatus of cyber risk assumes the harm lands on a balance sheet.
Artificial intelligence does not respect that boundary. When a model sits between a person and their health, their money, or their future, a failure doesn’t debit an account — it lands in someone’s life.
An algorithm discharges a grandmother from the care she still needs. A model denies a mortgage, or a job, to someone who qualified. A hallucinated dose reaches a patient. None of it is recoverable the way downtime is. You can restore a database from a backup. You cannot restore the six weeks of care a person didn’t receive, or the opportunity they were quietly screened out of.
The asymmetry is the cruel part. The person on the receiving end usually can’t see the decision, can’t audit it, and often can’t appeal it. In the litigation around one major insurer’s care-denial AI, an error rate as high as 90% was alleged — while only 0.2% of denials were ever appealed. When contesting a machine’s decision costs more than a person can bear, “the model was right most of the time” stops being a defense and becomes the whole problem.
“Move fast and break things” was survivable when the thing that broke was a feature flag. When the thing that breaks is a person, the calculus inverts. Governance, security, and explainability stop being compliance overhead and become the load-bearing structure of the product itself. Every consequential decision has to be provable, auditable, explainable, and accountable — not because a regulator demands it, but because a human being is on the other end of it.
That is why precision and trust aren’t features you bolt onto AI that touches real lives. In those domains, they are the entire point — and building as though that were true isn’t caution. It’s the job.
